For this particular feed, you should be using the attachment, or the 1st post image if you have full-text article extraction enabled. Alternatively, you can select "Insert attachments at the top of the post" from the "Media attachments" drop-down menu. All of these methods will work correctly.

Does it actually contain the image URL? Is it stored there using the "Custom fields" box on the "Advanced" tab of your feed settings? If not, you should select "Create from the first post image" in the "Post thumbnails" drop-down menu.

I've tried using AI generators set the custom thumbnail and use that, I've tried using first, last and random images from posts. I've tried using the media attachments and the thumbnail in the feed. Also tried FIFU but it is a no go. I've tried this with multiple feeds.

Does anyone have any insight into what might be missing?
